Notes

  • The top portion is for you -- the bottom is for us!! The dividing line is at
    -------Management------
  • Choose the Client Department (your department) from the drop down boxes. If there isn't a choice, use the "Specify your own"
  • Sub-department is mostly for large units with sub areas. Most of the management reports (step 3) will break by sub-department.
  • "Send e-mail" sends e-mail; there is a note on the field to further explain this.
  • You can add attachments to the request -- these may be e-mails or Word Documents or spreadsheets that further explain the request.
  • All requests go into a big pile, we review and assign them.
